We're seeking an experienced and accredited Mental Health Social Worker (MHSW) to join a growing, purpose-driven team supporting Australian Veterans through a coordinated, biopsychosocial model of care.
This is a hybrid role based in Springhill - QLD, primarily delivering telehealth consultations initially, with in-person sessions based out of our Enoggera Clinic and fortnightly group programs once established.
You'll be part of a highly collaborative multidisciplinary team, supported by strong clinical governance, structured care planning, and well-established systems.
About the Role
As a Mental Health Social Worker, you'll operate in an advanced mental health practice role, working from a biopsychosocial and person-in-environment framework. Your work will focus on psychosocial support, DMT-informed practice, assessments, and ongoing case management - building strong therapeutic relationships with Veterans across the lifespan.
Your Key Responsibilities
- Deliver evidence-informed mental health and psychosocial interventions to Veterans
- Conduct comprehensive biopsychosocial assessments and ongoing case management
- Build and maintain strong therapeutic relationships with Veteran clients
- Participate in Chronic Care Team Reviews alongside GPs and allied health clinicians
- Develop, implement, and review Care Plans for Veterans on your caseload
- Discuss Care Plans with Veterans and provide clear summaries to support engagement and outcomes
- Facilitate regular group therapy sessions (fortnightly - in person at Enoggera)
- Work collaboratively within a multidisciplinary Chronic Care Team to ensure coordinated, person-centred care
About You
You will be:
- A qualified and credentialed Mental Health Social Worker with the AASW
- Experienced in mental health assessment, intervention, and case management
- Comfortable working via telehealth within a structured care-planning framework
- Confident contributing to or facilitating group-based therapeutic programs
- Collaborative, reflective, and values-driven in your practice
- Passionate about improving outcomes for Australian Veterans
- Salary: $105,000-$115,000 + Super
- Hybrid working model after 3-4 weeks of training and onboarding (5 days WFH per fortnight)
